YOUTH BASKETBALL TRAINING
LIABILITY WAIVER & MEDIA RELEASE
Parent/Guardian Acknowledgment for Minor Participant
I, the undersigned parent or legal guardian, acknowledge and agree that my child’s participation in youth basketball training, skills development, conditioning, scrimmages, camps, clinics, and related activities involves inherent risks. In consideration of the minor participant being allowed to participate, I agree to the following:
1. Assumption of Risk
I understand that participation may involve risks including, but not limited to, falls, collisions, contact with other participants, physical exertion, equipment-related incidents, and injuries such as sprains, strains, fractures, concussions, dehydration, or other bodily harm. I voluntarily assume all known and unknown risks associated with participation.
2. Release and Waiver of Liability
To the fullest extent permitted by law, I release, waive, and discharge the youth basketball training organization, its owners, directors, coaches, trainers, staff, volunteers, sponsors, facility providers, and affiliates from any and all claims, demands, liabilities, damages, costs, or causes of action arising out of or related to the minor participant’s attendance at or participation in any training session, event, or related activity, including those occurring on indoor or outdoor courts or surrounding premises, except to the extent caused by gross negligence or willful misconduct where such release is prohibited by law.
3. Outdoor Activity Acknowledgment
I understand that some training or related activities may occur outdoors and may expose participants to additional risks, including heat, sunlight, wind, rain, slippery or uneven surfaces, environmental conditions, or other outdoor hazards. I accept and assume these risks on behalf of the minor participant.
4. Injury and Medical Treatment
I understand that injuries may occur during training and that the organization does not guarantee a risk-free environment. In the event of an emergency, I authorize the organization and its representatives to obtain reasonable emergency medical assistance for my child if I cannot be reached immediately. I understand that I am solely responsible for any medical expenses and that the organization does not provide medical insurance coverage for participants.
5. Photo and Video Release
I grant permission for the organization to photograph, record, and otherwise capture the minor participant’s image, likeness, voice, and performance during training sessions or events, and to use such materials for lawful promotional, educational, advertising, social media, website, and archival purposes without compensation. I understand that the organization may edit and publish such materials in print, digital, and online formats.
6. Optional Media Opt-Out
If I do not wish for my child’s photo or video to be used for promotional purposes, I will clearly indicate that choice below. I understand that reasonable efforts will be made to honor the request, but incidental inclusion in group or event footage may still occur.
7. Participant Health and Conduct
I represent that my child is physically able to participate in basketball-related activities and will comply with instructions, safety rules, and behavioral expectations established by the organization. I understand that the organization may remove a participant from any session for unsafe, disruptive, or inappropriate conduct.
8. Indemnification
I agree to indemnify and hold harmless the organization and the released parties from and against claims, damages, losses, or expenses, including reasonable attorneys’ fees, arising out of my child’s participation or my breach of this agreement.
9. Acknowledgment
I have carefully read this document, understand its contents, and sign it voluntarily. I understand that this is intended to be a legally binding waiver, release, and media authorization.
